The latest Bray Wyatt QR code which flashed on Smackdown led to a psychological/psychiatric evaluation report, most of which is redacted.

Filled by hand rather than the suggested word processing software in order to get reimbursement, the client’s name is listed as Wyatt, Bray with a date of birth of May 23, 1987 and a client ID of 855-211-1333.

The client ID is actually a phone number which when you call you get a voice mail with a hushed voice saying, “Oh no, not me. I never lost control.” The message is repeated twice before the phone hangs up.

The client’s signature in the form is listed as “refused to sign” and psychosocial history reads, “Difficulty showing remorse or empathy / conflict with social norms.”

A bunch of other stuff is redacted but in the Other section there’s written, “Disregarding or violating the rights of others.”

Some of the fields are redacted neatly with black boxes while other fields are redacted with a marker.
